What ongoing support will look like
We're still developing the exact format based on what we learn from our founding cohort — but the shape of it is clear. People managing complex chronic conditions don't just need a plan once. They need someone who knows their history and can show up as things shift.
Ongoing support from Groundwork will be for clients who've already done an initial consultation and want to continue the relationship — for provider changes, new diagnoses, appointment prep, or the kind of slow-moving coordination that nobody else is tracking.
We'll reach out to everyone on this list before we open applications publicly.
