Here's what working with us looks like.
We know you're tired. We've made this as simple as we could — four steps, no medical history required in the first form, no hoops to jump through just to see if we're a fit.
Four steps, start to finish
Fill out the intake form
About five minutes. We ask about your conditions, your situation, and what you're hoping for. No medical records, no detailed history — just enough to understand where you are.
~5 minutesWe review and confirm fit
We read every intake form personally. If we think we can help — or if we're not sure — we'll follow up within a few business days to talk through next steps.
2–3 business daysBook your initial consultation
A 90-minute video call. We go deep on your conditions, your history, and what you need. You'll get a written summary and a prioritized next-steps list.

Based on what we learn in the consultation, we figure out what makes sense next — whether that's a founding client package, a specific referral, or something else entirely.
No commitment requiredTell us about your situation.
This form is the first step — not a commitment to anything. We read it, we think about whether and how we can help, and we get back to you. Simple as that.
We don't ask for medical records here. We're just trying to understand where you are and what you need.
Submitting this form does not create a client relationship or constitute medical advice. Groundwork Advocates provides patient navigation and advocacy services only.
Intake form
Before you hit send
We're in our founding period, which means capacity is limited. If we're full when you reach out, we'll let you know and add you to the list. We'd rather be honest about that upfront than leave you waiting without context.
Fill out the intake form anyway. "Not sure where to start" is one of the most common answers we get to that question — and it's exactly what the initial consultation is designed for. You don't need to have it figured out before you reach out.
Yes. All services are offered via video call, so location isn't a barrier for most of what we do. Provider sourcing and research is applicable nationally. School advocacy may have geographic considerations depending on your state and district — we'll be upfront if that affects what we can do for you.
We read it — both of us, personally — and respond within a few business days. If we think we can help, we'll reach out to talk through next steps. If we're not the right fit, or if capacity is limited, we'll tell you that too. No automated replies, no form letters.
